AFFIDAVIT OF ELIGIBILITY

I understand that to be considered for the Grandwood Park Civic Association scholarship award that I must have established at least one year of residency in the area known as Grandwood Park in unincorporated Warren Township, IL and that my family is a current member of the Grandwood Park Civic Association.  I further understand that I must be, on April 30, 2009, a high school student or enrolled in a state accredited college, university, or technical/trade school and that I am a full-time student currently maintaining a minimum of 12 credit hours.  I must also have maintained a cumulative grade point average of at least 3.0.  I am submitting, along with this application, a copy of my high school or college transcript, which will attest that I have the required grade point average.

I also understand that I will be required to submit to a personal interview with members of the Scholarship Committee at a mutually convenient time prior to the selection of award recipients.  Interviews are tentatively scheduled for the third week in May.

I understand that the award check, if I am selected as a scholarship recipient, may be made payable to both myself as well as the school I am currently/will be attending.  This is to ensure that these funds will be used for the sole purpose of their intent, which is to provide me with financial assistance as I pursue my education.

I accept the policy of the Scholarship Committee that this application will be reviewed and evaluated based on my academic achievements and community service and that I have not been asked to declare my sex, race, or religious preference, nor have I been required to pay any monetary scholarship application fee.  Any investigation regarding the authenticity of this application will not involve disclosure of information related to my sex, race, or religious preference.

I further accept the fact that the decision of the Scholarship Committee is final and that I cannot hold any member of the Committee or the Grandwood Park Civic Association

liable should my application not be accepted.
